1. Application for Admission:
Complete an Application for Admissions for each Applicant (K-4 through 12).

2. Testing Appointments:
Grades 1 - 12: Call the Admissions Office for more information and appointments. Three items must be submitted in order to process an application and schedule testing:
New Student Testing Information Form
$50.00 non-refundable testing fee per student
A copy of the student’s most recent report card.
Testing Time: Approximately 2 hrs.

3. Entrance Requirements:
Students must score at the current grade level or above on Entrance Test.
Students must have an overall Stanine of 5 (total battery) on previous year’s Achievement Test
Student’s grades in core courses must be a passing level (70 average).
If a student is deficient in any of the three established criteria, admission may be granted on a probationary basis with the approval of Administration. Parents will be advised in writing of the probationary status

4. Information Required for Admission:
A photocopy of the certified birth certificate
Certificate of Immunization (Blue Form)

5. Course Selection Sheet:
Sixth through twelfth grade students should receive a class request sheet which should be filled out and returned immediately to insure that he / she chooses the required (and desired) classes. Please contact the Guidance Office if you have questions regarding course selections.

6. Contract of Enrollment:
The Business Office must receive a signed contract along with Registration Fees.

* Enrollment is considered complete only when all the necessary forms have been returned, all fees have been paid, and testing has been completed.
